โศกนาฏกรรมเมื่อ 10 ปีก่อน ทำให้เมืองฮาร์โมนีเปลี่ยนไปตลอดกาล เมื่อ ทอม ฮานนิเกอร์ (เจนเซน แอกเคิลส์) คนงานเหมืองมือใหม่ขาดประสบการณ์ ทำให้เกิดอุบัติเหตุขึ้นในอุโมงค์ จนมีคนเสียชีวิตไป 5 คน ผู้รอดชีวิตเพียงคนเดียว คือ แฮร์รี วาร์เดน (ริชาร์ด จอห์น วอลเตอร์ส) ซึ่งบาดเจ็บสาหัส

1 ปีต่อจากเหตุการณ์นั้น แฮร์รี ฟื้นขึ้นในวันวาเลนไทน์พอดี แล้วเขาก็ใช้พลั่วคู่ใจออกสังหารผู้คนถึง 22 ศพอย่างเลือดเย็น ก่อนที่จะถูกจับตายในที่สุด

ต่อมา ทอม ที่หนีออกจากเมืองไป 10 ปี กลับมายังเมืองฮาร์โมนีในวันวาเลนไทน์พอดีอีกครั้ง อดีตที่ผิดพลาดยังตามหลอกหลอนเขา อีกทั้งเขายังมีความรู้สึกดีๆ กับ ซาราห์ (เจมี คิง) แฟนเก่าที่ตอนนี้แต่งงานกับ เอเซล (เคอรร์ สมิธ) เพื่อนสนิทของเขาที่กลายเป็นนายอำเภอประจำเมือง

Ten years ago, a tragedy changed the town of Harmony forever. Tom Hanniger (Jensen Ackles), an inexperienced coal miner, caused an accident in the tunnels that trapped and killed five men and sent the only survivor, Harry Warden, into a permanent coma.

Then, exactly one year later, on Valentine's Day, Harry Warden (Richard John Walters) woke up and brutally murdered twenty-two people with a pickaxe before being killed.

Ten years later, Tom returns to Harmony on Valentine's Day, still haunted by the deaths he caused. Struggling to make amends with his past, he grapples with unresolved feelings for his ex-girlfriend Sarah (Jaime King), who is now married to his best friend Axel (Kerr Smith), the town sheriff.

But tonight, after years of peace, something from Harmony's dark past has returned. Wearing a miner's mask and armed with a pickaxe, an unstoppable killer is on the loose. Has Harry come from beyond the grave or is there a new killer donning the suit and mask?
